Why Some Exercises Make Sciatica Worse

Exercise is one of the best treatments for sciatica — but the wrong exercise can set you back by weeks. The sciatic nerve runs from your lower lumbar spine through your glutes and down each leg. When a herniated disc, bone spur, or tight piriformis muscle compresses that nerve, certain movements increase the pressure and amplify your pain.

The main mechanisms that make exercises harmful for sciatica are:

  1. Increased intradiscal pressure — Forward bending under load pushes disc material posteriorly, directly compressing the nerve root. Understanding the Sciatic Nerve

The sciatic nerve is the longest and thickest nerve in the human body. It originates from nerve roots L4 through S3 in the lower lumbar and sacral spine, converges into a single trunk about the width of your thumb, passes through or beneath the piriformis muscle in the buttock, and travels down the posterior thigh before splitting into the tibial and common peroneal nerves at the knee.

Sciatica — technically called lumbar radiculopathy — occurs when one or more of these nerve roots are compressed or irritated. The most common causes include:

  • Lumbar disc herniation (90% of cases) — The gel-like center of a spinal disc pushes through the outer ring and presses on a nerve root. The L4-L5 and L5-S1 levels account for roughly 95% of lumbar disc herniations (Kreiner et al., 2014).
  • Spinal stenosis — The spinal canal narrows with age, squeezing nerve roots.
  • Piriformis syndrome — The piriformis muscle in the buttock spasms or tightens, compressing the nerve as it passes underneath.

Each of these causes responds differently to exercise. A disc herniation is typically aggravated by flexion (forward bending), while spinal stenosis is often aggravated by extension (arching backward). This is why a one-size-fits-all exercise list is imperfect — but the exercises below are harmful for the majority of sciatica patients, particularly those with disc-related pathology.


The 10 Worst Exercises for Sciatica

1. Standing Toe Touches

Standing toe touches are the single most harmful exercise for disc-related sciatica. When you bend forward at the waist with straight legs, you simultaneously load the lumbar spine under flexion and put maximum tension on the sciatic nerve. Intradiscal pressure spikes dramatically in this position.

Many people stretch their hamstrings this way, thinking it will relieve sciatica. But tight hamstrings and sciatica pain in the back of the leg are two different problems. Stretching the nerve itself mimics the straight leg raise test — a clinical diagnostic tool that deliberately provokes sciatic pain.

2. Traditional Sit-Ups and Crunches

Sit-ups generate enormous compressive force on the lumbar spine. Research by McGill (2007) measured peak compressive loads of over 3,300 N during a standard sit-up — well above the threshold shown to cause disc damage in cadaveric studies. Each repetition flexes the lumbar spine under load, creating a pumping mechanism that can worsen disc protrusions.

The psoas muscle, a primary hip flexor heavily involved in sit-ups, also originates on the lumbar vertebrae. When it contracts forcefully, it pulls anteriorly on the spine, increasing compression on the posterior disc where herniations typically occur.

3. Double Leg Lifts

Lying flat and raising both legs simultaneously places extreme demand on the hip flexors and lower abdominals, but the lumbar spine pays the price. The pelvis tilts anteriorly as the legs rise, hyperextending the low back and compressing disc material against the nerve roots. People with weak core stabilization — which includes most sciatica patients in an acute flare — cannot maintain a neutral spine during this exercise.

4. Heavy Barbell Squats

Heavy back squats place significant axial load directly through the lumbar spine. Even with perfect form, compressive forces at L4-L5 during a heavy squat can exceed 10 times body weight (Cappozzo et al., 1985). If your sciatica stems from a disc herniation or stenosis, this compression can directly worsen nerve impingement.

The problem is compounded by the flexion that occurs at the bottom of the squat. Most lifters experience some degree of "butt wink" — posterior pelvic tilt at depth — which adds a flexion component to an already highly compressed spine.

5. Conventional Deadlifts

Conventional deadlifts combine the two worst elements for a compromised disc: heavy axial loading and lumbar flexion at the start of the pull. Even experienced lifters often round their lower back slightly during the initial pull from the floor, and when you add 100+ kg to that flexed position, the result is enormous posterior disc pressure.

A 2020 systematic review in the British Journal of Sports Medicine found that heavy lifting with spinal flexion was among the strongest occupational risk factors for lumbar disc herniation (Swain et al., 2020).

6. Seated Hamstring Stretch (Machine or Floor)

The seated hamstring stretch — whether on a machine or sitting on the floor reaching for your toes — is essentially a sit-and-reach test. This position maximally flexes the lumbar spine while simultaneously tensioning the sciatic nerve along its entire length. It combines neural tension and lumbar flexion in one movement.

If you need to stretch your hamstrings (and many sciatica patients do, because tight hamstrings contribute to posterior pelvic tilt and lumbar disc stress), do it in a way that doesn't flex the spine. Supine hamstring stretches with a strap or towel allow you to keep the lumbar spine neutral while lengthening the hamstring in isolation.

7. Burpees

Burpees combine every movement pattern that aggravates sciatica into one explosive exercise: deep squat, forward fold, plank jump-back, push-up, and explosive jump. The rapid transitions between flexion and extension under momentum give your spine no time to stabilize, and the impact loading from the jump adds compressive force on every rep.

8. Leg Press (Full Range)

The leg press machine forces the lumbar spine into flexion at the bottom of the movement as the knees approach the chest. Unlike a squat, where you can control depth and bail at any time, the fixed angle of the leg press locks you into a range of motion that may exceed your spine's tolerance. The heavy loads commonly used on this machine compound the problem.

9. Bent-Over Rows

Bent-over barbell or dumbbell rows require you to maintain a hip-hinged, forward-bent position while pulling heavy weight. This sustained flexion under load is precisely what increases intradiscal pressure. Even with a flat back, the isometric demand on the lumbar extensors to maintain that position while rowing heavy weight is substantial, and form breakdown is common as fatigue sets in.

10. High-Impact Plyometrics

Box jumps, jump squats, depth jumps, and bounding activities create repetitive impact forces that transmit directly through the spine. Each landing generates ground reaction forces of 3-6 times body weight, and those forces must be absorbed by the vertebral column. For a spine with an active disc herniation or stenosis, this repeated shock loading can worsen symptoms significantly.

How to Modify Your Workout With Sciatica

You do not have to abandon the gym entirely during a sciatica flare. The key is modifying exercises to reduce spinal flexion, compression, and neural tension.

General Modification Principles

  • Reduce load — Drop weights by 40-50% and focus on controlled movement. Lower loads mean less spinal compression.
  • Limit range of motion — Partial squats (above parallel) and partial deadlifts (rack pulls from knee height) remove the flexion component at depth.

  4. Return to full training: Only when you can perform all daily activities pain-free for at least 2 consecutive weeks

Methodology

Exercise recommendations in this article are based on biomechanical principles of spinal loading, neural tension mechanics, and clinical evidence from randomized controlled trials. Exercises were categorized as "avoid" based on their potential to increase intradiscal pressure beyond safe thresholds, place the sciatic nerve under sustained tension, or create compressive forces on compromised spinal segments.
